Birthday Club Form.  Celebrate your child’s birthday with a gift all of the children at school can enjoy!  For a contribution of $15 or more, your child will have a brand new book dedicated in his/her name in the school media center. A permanent label will be affixed to the book to honor your child’s growth and contribution, and your child will receive a happy-birthday certificate as well. Half birthdays can be celebrated for the summer born! Fill out and return the form on the back of this sheet to include your child in the birthday fun!

Carpool Map.
Covenant of Learning Document.  The purpose of this Covenant of Learning Document is to establish a commitment between home and school. This covenant is to be based upon mutual trust and the shared vision that the Community School of Davidson will be a community of learning and committed to excellence. It is imperative that parents and school be true partners in educating our children.  This document is signed each year by our parents and students and turned in during Covenant Conference time in August.
Davidson Walks and Rolls Permission Form.  CSD is proud to participate in the Davidson Walks and Rolls program.  The mandate of DavidsonWalks & Rolls is to create safe routes for students and parents who wish to walk and bike to school.  To learn more click here.
Educational Opportunity Form (for travel).  Going out of town on an extended trip during school and would like it to be an excused absence for your child?  You’ll need to fill this form out and turn it into the front desk for administrator approval! 
Enrollment Intention Form.  Each family will fill out this form during the second half of the year to let us know what your plans are for the following year.

Exceptional Needs Form.  We ask that each new family fill out one of these form in detail and in its entirety.  If this does not apply for your child, we still ask that you turn this in indicating it is not applicable.

Medication Form.  If your child requires medication to be administered during school hours by a staff member, this form needs to be filled out in its entirety.

Student Permission Form.  Upon admittance to our school, each child is required to turn a student permission form in to us.  Each family will fill this form out once upon enrolling in our school.  Changes to preferences can be resubmitted to us at any time.

K - Heath Assessment Form.  All kindergarteners are required to have this complete prior to the first day of school.  Pay particular attention to the back page.  You must attach a copy of your immunization records. 
Parent/Student Survey.  To be filled out and turned in during Covenant Conferences at the beginning of each school year.

Athletics

Spartan Athletic Booster Club Form.  The objective of the Spartan Athletic Booster Club is to encourage and to support the athletic activities of the Community School of Davidson. This objective is accomplished through financial, as well as moral support. All funds raised are used to directly benefit the student athletes. 
Sports Physical Form.  Required if your child will be participating in any CSD sports.
