Celebrate your child’s birthday with a gift all of the children at school can enjoy! For a contribution of $15 or more, your child will have a brand new book dedicated in his/her name in the school media center. A permanent label will be affixed to the book to honor your child’s growth and contribution, and your child will receive a happy-birthday certificate as well. This covenant is to be based upon mutual trust and the shared vision that the Community School of Davidson will be a community of learning and committed to excellence. It is imperative that parents and school be true partners in educating our children. This document is signed each year by our parents and students and turned in during Covenant Conference time in August. Davidson Walks and Rolls Permission Form. CSD is proud to participate in the Davidson Walks and Rolls program. The mandate of DavidsonWalks & Rolls is to create safe routes for students and parents who wish to walk and bike to school. You’ll need to fill this form out and turn it into the front desk for administrator approval! Enrollment Intention Form. Each family will fill out this form during the second half of the year to let us know what your plans are for the following year. ESL Form. Elementary School specific forms
K - Heath Assessment Form. All kindergarteners are required to have this complete prior to the first day of school. Pay particular attention to the back page. Athletics
Spartan Athletic Booster Club Form. The objective of the Spartan Athletic Booster Club is to encourage and to support the athletic activities of the Community School of Davidson. This objective is accomplished through financial, as well as moral support. All funds raised are used to directly benefit the student athletes. Sports Physical Form.
